Interventional orthopedics is a newer branch of orthopedic medicine that centers on the use of advanced image-guided techniques, such as ultrasound or fluoroscopy, to treat musculoskeletal conditions through precisely targeted injections that previously were not possible. These treatments are minimally invasive, thus allowing for lower risk of complications, quicker recovery, and improved functional outcomes. Orthobiologics and other forms of regenerative medicine are often intertwined with interventional orthopedics.

Conditions We Treat Using Interventional Orthopedics

We offer non-surgical treatment for both degenerative and active acute orthopedic conditions. Degenerative orthopedic conditions affect the musculoskeletal system and often result from wear and tear of tendons, joints, and ligaments over time or due to chronic overuse. The prevalence of degenerative conditions increases with age and is usually progressive and chronic. 

At Impact Sports & Spine, interventional orthopedics involves using evidence-informed, advanced procedures to target the root cause of degenerative orthopedic conditions. 

Common degenerative orthopedic conditions we treat include:

  • Osteoarthritis: Osteoarthritis is the most common type of arthritis, primarily affecting the joints due to the breakdown of cartilage and the underlying bone.
  • Tendinopathy: Tendinopathy involves the deterioration or architectural disruption of tendons due to repetitive stress or injury. 

Unlike degenerative conditions, active acute orthopedic conditions arise suddenly from traumatic injuries or accidents. These can include sprains and strains, which may necessitate immediate medical attention to prevent further damage and promote healing. We offer non-surgical, targeted procedures to treat acute orthopedic conditions.

Interventional Orthopedic Treatments We Offer

Interventional orthopedics uses image-guided techniques, such as ultrasound or fluoroscopy, for the precise treatment of the affected areas. Our treatment aims to address the underlying causes of your conditions rather than just alleviating your symptoms.

Joint Injections

Joint injections may help people with arthritis or other inflammatory conditions by injecting medications directly into the affected joints to reduce pain and swelling. We administer ultrasound and fluoroscopy-guided joint injections to ensure precise needle placement and maximize the effectiveness of the injection while minimizing risks.

Joint injections can include corticosteroid medications to reduce inflammation and hyaluronic acid-based medications to improve joint lubrication and mobility.

Corticosteroids are fast-acting medications that mimic the effects of cortisol, a hormone naturally produced by the adrenal glands, to suppress immune responses and inflammatory processes at the cellular level.

Hyaluronic acid (HA) is a natural lubricant that helps the bones in joints slide over each other. It is especially helpful in chronic knee pain.

Peritendinous Injections

Peritendinous injections target the area directly adjacent or next to, a tendon. Without the use of advanced imaging, such injections are not possible as the needle may inadvertently be advanced directly into the tendon. Depending on the substance being injected, this could result in further injury and pain. Peritendinous injections can be considered for the treatment of conditions such as tendinitis and tenosynovitis.  

For other tendon-related conditions, direct injection of the tendon may be desirable, particularly if an orthobiologic is being utilized. In such an instance, image-guided techniques are crucial for accurate needle guidance and injectate delivery. 

Ligament Injections

Injured ligaments may also be a target of interventional orthopedic injections. Similarly to tendon-related injections, image guidance is critical for ensuring accurate needle placement either along or directly into a ligament. Ligament injections may be indicated for the treatment of ligament sprains, chronic ligament pain, and joint instability. 

Commonly used therapies include Platelet-Rich Plasma (PRP) injections, which utilize the patient's own concentrated platelets to accelerate tissue repair, and prolotherapy, which involves injecting a dextrose solution to stimulate the body's natural healing processes. 

Fascial Injections

Fascial injections are a targeted treatment used to relieve pain and discomfort associated with fascial tears and/or fascial thickening. Such pathology can only be seen through the use of advanced, high-resolution ultrasound. Once confirmed, these small tears or thickened areas, can be precisely targeted to alleviate pain and dysfunction.  

Peripheral Nerve Injections

Peripheral nerve injections target various nerves throughout the body. Without the use of high-resolution ultrasound, such injections are not feasible. With ultrasound guidance, nerve entrapments can be identified and precisely treated to alleviate symptoms such as pain, numbness, and tingling.  

Epidural/Nerve Root Injections

Epidural injections are a common and effective treatment for managing chronic pain in the spine, including the lumbar, thoracic, and cervical regions. 

These injections are delivered into the epidural space around the nerve roots and spinal cord, targeting inflammation and pain caused by conditions such as herniated discs, spinal stenosis, and degenerative disc disease. 

Epidural injections, administered under fluoroscopic or ultrasound guidance for precise placement, help reduce pain, decrease inflammation, and improve overall function. Selective nerve root injections are similar to epidurals; however, the nerve root is targeted further away from its origin. The goal of these injections is to alleviate pain caused by irritated or compressed spinal nerve roots. 

Intradiscal Injections

Intradiscal injections deliver medication directly into the affected disc in your spine to reduce inflammation, alleviate pain, and promote healing. These injections are guided by fluoroscopy to ensure precise placement of the needle into the affected disc.

These injections offer a targeted, minimally-invasive treatment option for patients suffering from back pain that often stems from the intervertebral discs.

Shockwave Therapy

Shockwave therapy is a non-invasive treatment that utilizes high-energy sound waves to stimulate the body's natural healing process. It can be used to treat a variety of musculoskeletal conditions.

Shockwave therapy triggers the release of growth factors, leading to tissue repair and the promotion of healing. It also promotes reduced pain through a process called neuromodulation. This non-invasive treatment option can offer a safe and effective way to manage pain and promote healing.

Shockwave therapy can be administered through two forms: radial shockwave therapy and focused shockwave therapy.

Radial shockwave therapy uses a lower-energy acoustic wave that spreads out in a radial pattern from the point of application. The radial wave disperses across a wider area, making it suitable for treating larger areas of soft tissue.

Focused shockwave therapy uses high-energy sound waves to target specific areas of the body. Unlike radial shockwave therapy, which has a wider dispersion pattern, focused shockwave therapy delivers a concentrated beam of energy to a precise location.
