New Patient Information

At Impact Sports & Spine, we prioritize making the journey from consultation to treatment as seamless and supportive as possible for our new patients. 

From the moment you contact us, our friendly and knowledgeable staff will guide you through the scheduling process, ensuring you have all of the required information and documentation ready for your first visit. You can also contact us if you have any questions. 

What can I expect during my first visit?

Once you have registered and scheduled an initial appointment, you will receive a notification about registering for our patient portal. It is important to complete this registration, as we regularly make use of the patient portal to provide you access to your office visit notes, orders, and important documents. Further, the portal enables you to have direct access to messaging your provider.  

Three days prior to your appointment, you will receive a text message reminder for your appointment. On this reminder, there will be a link that allows you to check-in for your appointment. It is important to access this link, as it will allow you to complete your new patient forms and questionnaires. Completing these forms in advance of your appointment is critical to allow for more time spent with your treating physician. If you experience any difficulty accessing the portal or receiving appointment reminders, please let us know.

During your first appointment, you will meet with one of our double-board-certified physicians for a comprehensive evaluation that helps pinpoint the root cause of your symptoms. 

This session may include reviewing your medical history, performing a thorough physical examination, and reviewing relevant imaging tests. Additionally, we may perform an advanced diagnostic ultrasound scan and ask for further tests (when needed) to make an accurate diagnosis. 

We will then discuss your goals and preferences to create a personalized treatment plan.

What should I bring to my first appointment?

Please bring any relevant medical records, imaging studies, and a list of your current medications. Additionally, having your insurance information and a form of identification will help expedite the check-in process. Much of the check-in process can be completed digitally in advance of your appointment.

How are you different from other clinics, especially stem cell clinics?

Unlike many stem cell clinics that rely on aggressive advertising and offer treatments with little medical evidence, we provide evidence-informed, FDA-compliant treatments for your musculoskeletal and spinal conditions. 

All of our treatments are performed by fellowship-trained, board-certified physicians, ensuring the highest standard of patient care and safety. Moreover, while other practices may rush to treatment, we take sufficient time to understand your condition and its underlying causes to plan a targeted treatment.

Do you accept insurance?

Yes, we accept a variety of insurance plans. We recommend contacting us to verify that we accept your specific insurance provider and to understand any potential out-of-pocket costs.
